Juliet Jacobsen, MD, DPM, is an Associate Professor of Medicine at Harvard University and serves as the medical director for the Continuum Project, a Massachusetts General Hospital (MGH) culture change initiative to improve the medical experience of patients, families, clinicians, and staff through engagement, advance care planning, and innovation in education and technology.

In 2006, she joined the faculty of the Division of Palliative Care and Geriatric Medicine at MGH and Harvard Medical School and was the fellowship director for the Harvard Interprofessional Palliative Care Fellowship for seven years. Juliet more recently has worked as a fellow at Children’s Health Defense.
